The forms are located on the emergency response station. Staff should include information such as date, time, and location of the incident, as well as the names of individuals involved or affected.

Descriptions of what transpired, any injuries sustained, and witnesses present should be thoroughly recorded. Additionally, any actions taken in response to the incident such as firs aid administered or equipment checked, should be documented.
